The Care Coordinator works with patients, families, and healthcare providers to ensure comprehensive, coordinated care across various services and resources. This role is essential in managing patient care plans, promoting health literacy, and helping patients navigate the healthcare system.
Key Responsibilities:
- Develop and oversee individualized care plans for patients.
- Coordinate communication between patients, families, and healthcare teams.
- Provide education on treatment options, resources, and preventative care.
- Assist patients in managing appointments, referrals, and follow-up care.
- Monitor patient progress and adjust care plans as needed.
Qualifications:
